Getting an Expert Cancer Second Opinion

Getting an oncology second opinion is relatively straight forward. How you go about this needs some careful consideration as it may have a negative effect on your cancer care or cancer prognosis.

Probably the easiest method is to ask your cancer specialist to refer you to another cancer specialist. Your cancer specialist is most likely to know who is most qualified to review your case. Although this is the easiest method it needs careful consideration the following reasons. There is no guarantee your cancer specialist will send you to the most appropriate doctor to review your case. They could just send you to someone they know or trained with. If this is the case, this doctor is likely to share the same approach to cancer medicine as they do. They are also likely to refer you to someone local or even at the same hospital. Although this may be most convenient for you, it may not be the best second opinion. The trust between yourself and your cancer specialist may be effected by asking for a second opinion. Although every cancer specialist wants you to be happy with the treatment plan, at a sub-conscious level the doctor patient relationship may be effected. Most importantly asking for a second opinion in this way may affect your prognosis as it may cause delays in you receiving your cancer treatment. There are very strict guidelines as to time scales from initial referral, diagnosis and treatment. Sometimes after the patient has had their second opinion, the cancer has developed or spread and the treatment can turn from radical (curative) to palliative.

You can also ask your GP to refer you to another cancer specialist. This is also fairly straight forward, but again has some disadvantages so needs careful consideration. The GP is unlikely to know the best cancer specialist to send you to. If you are going to ask your GP for a second opinion referral, then it is best you do your own research to find who you would like to be referred to. You could then ask the GP to refer you to a specific specialist. How you approach the research will be challenging. Even if you find a doctor who you believe is 'the best' they may not accept or have the capacity in their busy schedule to see another patient. Therefore, if you do choose this route then take a couple of options to your GP to avoid disappointment. Like with the above option, there is a real risk that delays will occur in your treatment pathway that may affect your prognosis.

Getting a private second opinion from the leading doctors in the country is a sensible approach, but you will of course have to pay for this. The advantage is that they will know who is considered the leading doctor treating your type of cancer. They will also be able to give you independent information on every aspect of your treatment so far and what options are available to you, going forward to the future. Some services will provide you with information regarding clinical trials that you may be eligible for and even search for trials internationally. Some second opinion services will also supply you with the information you were given in the consultation that you can take away the same day.

The 4th option is not to get a second opinion. You may be happy with the treatment plan proposed by your cancer specialist. Most cancer patients are discussed in a multi-disciplinary meeting (MDT) so it is likely that you will offered treatment that conforms to national guidelines.

Whatever decision you make will be the right decision as long as you have given it adequate thought.

Breast Cancer Awareness Is Only a First Step

One in eight US women will be diagnosed with breast cancer. Overcoming breast cancer is a personal journey with choices along the way. There are choices about the type of breast surgery, the need for additional treatments and options for breast reconstruction. Research has found that breast reconstruction contributes to a woman's self-esteem and sense of wellbeing; however, more than 70 percent of women eligible for breast reconstruction are not told about all of their options.

The media frequently heightens awareness of breast cancer, but awareness doesn't always translate into a positive impact on health care. Agreeing or disagreeing that Angelina Jolie Pitt's decision to surgically remove her ovaries and fallopian tubes to prevent ovarian cancer was a heroic and courageous move is a matter of perspective. However, in between the lines of Angelina's latest article in the NY Times ("Angelina Jolie Pitt: Diary of a Surgery", NY Times, March 24, 2015) lies a subtler message... Every woman diagnosed with breast cancer should seek a referral to a plastic surgeon to learn about all of her reconstructive options before her cancer is treated and removed. Breast reconstruction after mastectomy or lumpectomy generally falls into two categories: implant based reconstruction or reconstruction using a person's own tissue, also known as autologous reconstruction. Can Sunscreen Lower Melanoma Risk Effectively?

Statistics show that in four decades the cases of skin cancer have tripled. This naturally poses the question for protecting the skin from the UV rays of the sun as they can cause DNA changes in cells and lead to the growth of tumours. Sunscreen products are widely used, but it is important to have an idea of how well they can protect you and how they can lower melanoma risk. How Sunscreen Works

This type of topical treatment has ingredients which work to prevent the penetration of the sun's UV rays into the skin. The different ingredients work in two main ways. Research studies on the link between the use of sunscreen and lower melanoma risk have shown mixed results. Some of the most recent ones are very promising. A study conducted in 2011 among 1,600 showed that the application of sunblock alone can reduce the risk by as much as half.

However, there are also studies which have shown that this product may actually increase the risk of the condition. This may be due to the fact that people tend to stay longer hours in the sun when they apply sunscreen. Generally, there is no solid evidence that the use of this type of product alone can reduce the risk.

Factors for Effectiveness

The effectiveness of the protective topical treatment depends not only on the product, but on the way in which it is applied. Experts suggest that the product has to be used every day irrespective of the weather outdoors. It has reapplied every half an hour or so during outdoor activities, especially when the person is sweating. Similarly, it has to be reapplied after a swim.

Conclusion

Overall, sunscreen alone may help to reduce melanoma risk, but in order for it to produce the best results it has to be combined with other protective measures. These include avoiding the sun during the hours of the day when the UV radiation is the strongest and covering as much of the body as possible with clothing.

The Best Foods in the Fight Against Cancer

The possibility of developing cancer is frightening to everyone, and indeed the odds of it happening are now one in three. Furthermore, the chance you might get it has increased over the past few decades. Fortunately, with advances in medicine, and increased knowledge of diet and nutrition, your chances of surviving it are now much greater than they were only a few years ago.

Few people realize that a violent struggle is going on in their body between molecules that are trying to give them cancer, and molecules that are trying to protect them from it. Every day thousands of cells in your body are forced into a pre-cancerous state by carcinogens. If they aren't repaired, some of them will eventually develop into full-blown cancer cells. Is there anything you can do to help prevent this? Indeed, there is, and one of the most important things is related to your diet. It is now known that a class of vegetables called cruciferous vegetables is extremely effective in protecting you from developing cancer. Some of them are: broccoli, Brussels sprouts, cabbage, cauliflower, kale, radishes, and turnip greens.

Attention was drawn to these vegetables in 1996 when articles in two different journals showed that they appeared to be extremely effective in fighting cancer. The first, in Cancer: Epidemiology, biomarkers, and Prevention, reviewed 94 studies on cancer prevention and concluded that consumption of cruciferous vegetables led to a significant decrease in the risk of both primary and secondary cancers. The second, in the Journal of the American Dietetic Association, showed that 70-percent of the studies it reviewed also indicated a close link between the consumption of cruciferous vegetable and protection against cancer. Numerous more recent studies have verified these results.

How and Why Crucifers Attack Cancer Cells

The major reason crucifers are so effective is that they contain what is called glucosinolates. They are sulfur-containing chemicals that give these vegetables their pungent odor and bitter taste. During cutting, chewing and digestion glucosinolates are broken down into several biologically active compounds, including indoles, nitriles, thiocyanates, and isothiocyanates. Of these, isothiocyanates (usually called ITC's) and indoles are the most important. Studies have shown that both inhibit the development of cancer in several different organs in rats and mice. They include the bladder, breasts, colon, liver, and stomach. These studies have also shown that cruciferous vegetables are helpful in preventing cancer in several different ways, including:

• They act to stop carcinogens from initiating cancer.
• They protect cells from DNA damage (which cause mutations that create cancer).
• They help inhibit tumor blood vessel formation (called angiogenesis).
• They help stop the migration of tumor cells to distant parts of the body (called metastasis)
• They induce cell death (apoptosis) when needed to get rid of cancer cells.

Most of the above studies were done using animals or cells grown in the lab. Almost all of them showed a benefit of cruciferous vegetables in helping to prevent cancer. Studies with people have also been made and they are encouraging, but some of them have mixed results, ranging from little effect to considerable effect. The reason for this is that there are problems when people are involved. First of all, studies of this type usually rely on people's memory of what they ate, and secondly, people who eat large numbers of crucifers usually have other behaviors that reduce their risk of cancer. Finally, it's well-known that genes also have an effect.

One of the best ITC's is what is called sulfurophane. It has been shown to prevent carcinogens from binding to DNA, and it also produces enzymes that repair DNA that has become damaged by a carcinogen. In all, about 120 ITC's have been identified, and they play many different roles in protecting DNA and stopping cancer.

How to Get Maximum Effect

Chopping, blending and breaking up of these vegetables is particularly important. This enhances the production of ITC's; in fact, the greater the breakup, the greater the number of ITC's produced. It is also best to eat them raw. If cooking is used, it is best to cut them up first, then cook them for only a short time. If they are cooked until they are mushy much of the benefit is lost. It's also important to chew them thoroughly. This applies particularly to broccoli and cauliflower. Also, in this case, it's important to remember that most of the nutrients are in the florets.

Cruciferous vegetables are also most effective when eaten together. So putting several of them in a salad is a good way to serve them. Juicing is also a good way of breaking them up, and juices of various vegetables can easily be mixed.

In addition to their role in protecting you from cancer, cruciferous vegetables also contain several carotenoids, including beta carotene, lutein and zeaxanthin which are particularly good for your eyes. Furthermore, they are a good source of vitamins C, E, folate and several minerals. And they are a good source of fiber.

How Many Should You Eat?

Two servings a day along with 5 servings of other vegetables is usually considered sufficient. Several servings of fresh fruit is also important. The National Cancer Institute recommends 5 to 9 servings of fruit and vegetables per day for cancer prevention.

Signs and Symptoms That You May Have a Brain Tumor

In 2012, 13,700 people died from brain or nervous system cancer and there were 22,910 new cases reported - that's just one year. So many people are impacted by cancer whether as a patient or a relative or friend of a patient; because of this, people are always concerned that they will become one of those statistics.

So what do you need to know?

Let's start with what a tumor actually is. Basically, tumors are abnormal cells that grow at a faster rate than normal cells. A tumor can be benign (meaning not malignant) or malignant ([muh-lig-nuhnt] meaning cancerous)

There are many different types of brain tumors. First, primary brain tumors are tumors that begin in the brain (not traveling from another part of the body). Any primary tumor refers to the type of tissue where the tumor first appeared. The most common brain tumors are call gliomas (that's pronounced glahy-oh-muh). A secondary tumor is one in which the cells travel from another area of the body - so a in the brain, a secondary tumor would be cells that traveled into the brain (via blood stream) and burrowed in and started multiplying cells in its new home.

There are a couple types of gliomas:

Astrocytomas (pronounced ās'trō-sī-tō'mə)- this is a cluster of star-shaped cells referred to as astrocytes. These can grow anywhere in the brain or spinal cord; but there are general pattern that seems to occur. Astrocytomas are most often found in the brain for adults but in the brain stem in children.

Most people know that cancer has many grades - I, II, III, and IV. Identifying cancer early allows a patient to be treated quickly and more effectively.

Anaplastic astrocytoma: This is a grade III astrocytoma.

Glioblastoma multiforme (pronounced glee-o-blas-TO-ma multih-FOR-may): This is a grade IV astrocytoma.

My brother died from a grade IV, glioblastoma multiforme, when he was 49 years old. We didn't know the signs that indicated he was in trouble. Obviously, we wish we had. Now I tell people what signs to look for so that they can get treatment in time and not go through the ugliness of dying from a brain tumor.

The most frequent symptoms of brain tumors include:


Headaches that tend to be worse in the morning and ease during the day
Seizures (convulsions) - what my brother experienced was little spaces of time that he couldn't account for. For example, while ordering food in a restaurant, he would suddenly stop talking - that was the extent of his seizure for many years until he had a grand mal seizure (that's the kind of seizure with convulsions).
Nausea or vomiting
Weakness or loss of feeling in the arms or legs
Stumbling or lack of coordination in walking (ataxic gait)
Abnormal eye movements or changes in vision
Drowsiness
Changes in personality or memory
Changes in speech: My brother lost his ability to speak well; this was because the tumor was in his speech center in his brain. This became one of our biggest clues to his condition.


Don't overlook any of these signs or symptoms in the people you love. Educating yourself and being observant may just help someone you know get the help they need before it's too late.

Do I Have A Prolactinoma?

A Prolactinoma is the most common type of benign brain tumor. This type of tumor is commonly misdiagnosed. People normally may not present with a headache, but they have other systemic symptoms instead. Some patients present with sore lactating breasts that produce milk. Women may also experience changes in their menstrual cycles, such as shorter or delayed menses. Later, people will experience changes in their vision. Patients may present after automobile accidents or clumsiness when they were hit from the side because they had developed Bilateral Hemianopsia, where they lost their side vision.

Patients are diagnosed with a Prolactinoma by having high serum Prolactin levels. They are then confirmed with a MRI showing a growth of the Anterior Pituitary, which sits in the sella turcica of the brain. The sella turcica sits on top of the Optic Chiasm, which is being compressed causing the visual disturbances.

The Anterior Pituitary not only produces Prolactin, but it also produces five other hormones that may be diminished with the overproduction of Prolactin from the gland. These other hormones are Luteinizing Hormone (LH) and Follicular Stimulating Hormone (FSH), which produce progesterone and estrogen in women and testosterone, Mullerian Inhibiting Factor (MIF) and Androgen Binding Protein (ABP) in men. Adrenocorticotrophic Hormone (ACTH) is also produced, which causes the release of Cortisol from the adrenal gland. Thyroid Stimulating Hormone (TSH) comes from the Anterior Pituitary, which causes T3 and T4 thyroid hormones to be released from the thyroid gland. Growth Hormone(GH) is also produced which acts on all cells in the body for growth and repair and stimulates the release of Insulin-Like Growth Factor (IGF) from the liver to help maintain the body's blood glucose levels. If the Prolactinoma is not treated, the Anterior Pituitary gland will not produce enough of these hormones to act on the other body organs to function properly, and patients will experience symptoms of decreased sexual functioning, Cushing's Syndrome, Hypothyroidism, and Hyperglycemia.

There are a couple treatment options for a Prolactinoma. The first treatment option is medication. The best medical option is Bromocriptine. It is an older drug that has been used for many years, but it has several side effects, such as chest pain, confusion, and low blood sugar, that some patients cannot tolerate. So the alternative medication that can be prescribed instead is Carbergoline. These drugs are dopamine agonists that work to reduce the amount of Prolactin that is produced from the Anterior Pituitary in the brain. The ultimate treatment of a Prolactinoma is surgery. The procedure is surgical removal of the brain tumor out through the nose of the patient.

The prognosis of a Prolactinoma is very good. Ninety-five percent of the patients with this benign tumor have functional lives with few side effects. If the tumor is small, women will still be able to get pregnant and have children. Because Prolactin antagonizes estrogen release from the ovaries, women have an increased chance of getting Osteoporosis, so patients should be followed by their primary care physician and/or Neurologist to monitor the growth of the Prolactinoma. Luckily, Prolactinomas do not grow much after five years, and patients can determine their long-term treatments at that time.

How to Determine That You Have Brain Cancer

Just under 1% of the human race will be susceptible to being diagnosed with either brain cancer, or another cancer associated to the nervous system sometime during their life-time. Put in layman's terms, that is 1 in every 161 men and women throughout the world will in some way be affected by the disease.

When trying to determine whether someone has brain cancer or not, it helps to have a basic understanding of what exactly the brain is, and what role the brain plays in the body.

The brain is the control center of the nervous system (the computer), which is made up of soft tissue (60% fat), and where the cerebral (the main part of the brain) has between 15 - 35 million neurons ([nerve cells] a core component of the nervous system) that are each connected to synapses (structures) that permit electrical or chemical signals to be passed to other cells.

The brain controls what someone does or does not do, by using a highly complex message system that passes messages backwards and forwards via a network of nerves. These may be things that someone is conscious of, such as walking and talking, or things that the body does automatically, like breathing and reflexes, etc. So when abnormalities exist within the brain, they tend to show-up in other parts of the body.

Typical brain cancer symptoms may include such things as: visionary problems, weakness, and paralysis (a stroke) due to the loss of brain cells, although symptoms can vary considerably, and may well depend on a patient's specific problem. When the brain tumor presses on the nerves, a sufferer may well experience problems such as not being able to think straight, such as with Alzheimer's disease.

Apart from the general tests to check whether someone has brain cancer, such as: blood, urine, diagnostic, lab, and pathology tests, three other major tests are used to conclude whether someone has the disease.

1. CT Scans (Computed Tomography) - a widely used high-resolution three-dimensional imaging scanner that uses X-rays to create cross-sectional images.

2. MIR Scans (Magnetic Resonance Imaging) - used after a contrast (a special dye that makes certain areas of the body show-up clearer) has been introduced into the body. The MRI scan uses powerful magnets and radio waves which are then translated into images by a computer.

3. PET Scans (Positron Emission Tomography) - a specialized imaging technique that uses radioactive substances to produce three-dimensional images.

In most cases, some type of treatment will be offered to brain cancer patients, either to correct the problem, or to try to improve its functionality. Physical therapies, medicines, and surgery are usually the most common treatments offered for sufferers of brain cancer, and other cancers related to the nervous system.

So, if you are in a space where you need strength, take a deep breath and feel a surge of oxygen entering your body and giving you life force energy. Do it again and you will feel even better, lighter, and more comfortable. When I find myself feeling down, I say, "Hello down (or sadness or fear or whatever the emotion may be). I am aware of you." Staying stuck in that negative place feels icky so I make a conscious decision to shift my mental, emotional, physical state and it works! I feel better, I smile, and I thrive!

Taking your mind away from the details of the health crisis to pleasure, fantasy, and enjoyable stimuli also helps shift you out of a negative state and build your strength. It is important to engage in activities that enhance your quality of life such as reading, attending movies and concerts, and getting plenty of laughter. Set aside the health crisis and all the decisions, questions, and concerns for a period of time. Listen to music, a hypnosis CD, or something else that brings you pleasure and relaxation. When you do this your body relaxes, your emotions calm, and your mind quiets. This restores the inner strength and personal power that keeps you going, keeps you positive, and keeps you motivated to live well even in the midst of a health challenge.

Melanoma can be broken down into three categories:

1. Cutaneous Melanoma (of the skin) - is where most pigment cells are found to be present in the skin, and is considered the most common of the three types. However, it can be further broken down into four more groups: superficial spreading, nodular, acral lentiginous, and lentigo melanoma.

2. Mucosal Melanoma - is the second type that affects any of the body's mucous membranes, including: the nasal passages, throat, vagina, anus, and the mouth.

3. Ocular Melanoma (uveal/choroidal melanoma) - is the third type, and considered the most rare of the three (occurring in the eyes only).

However, nodular melanoma (although one of the most common), is the most aggressive form of all. It is able to grow in a matter of months (even weeks), as its metastasis (spread) grows vertically over the skin in the form of small black, or pink (if amelanotic [showing no dark pigment at an early stage]) malignant (cancerous [tumor]) nodules that tend to bleed as they enlarge.

Other symptoms may include: dome-shaped bumps (black, brown, red, or blue), shiny, scaly, or oozing bumps, and tender, itchy swellings that have the tendency to bleed (all indicate the possibility of the presence of a tumor).

Causes of nodular melanoma - are usually over exposure to sunlight radiation; however, exceptions may include the vulva and the sole of the foot where sun radiation does not affect, although nodules may still appear.

Diagnosis of nodular melanoma - is usually through inspection, followed by biopsy (a sample of tissue is taken for analysis under the microscope).

Treatment for nodular melanoma - may depend mainly upon whether there are any other regions of the body affected. However, treatment usually consists of the following: surgical removal (excisional biopsy, wide local excision, and possibly sentinel node biopsy), chemotherapy, and radiation therapy.

3 Things Children of Cancer Patients Are Tired of Hearing

When my dad was diagnosed with cancer a month before my freshman year at college, it came like a freight train to the gut. There is nothing more frightening, in my experience, than watching your parents sit down on the couch, hands clutched together tightly, and begin to sob in earnest.

Somewhere in the emotional melee, they manage to tell you that they have some news, and if you're smart and have been paying attention, you know that whatever it is cannot possibly be good. They let the dreaded word fly and I immediately left the house to take the dog on a three hour walk (or stumble, in my case) around the neighborhood.

Plain and simple, it sucks. It really, really sucks to watch the man who chased the monsters from under your bed be tackled and slowly devoured by a creature you never thought you'd have to see.

And ironically, as a Christian, what sometimes sucks even more is letting other people in. Everyone always has something to say, good or bad, and sometimes, as well-meaning as they are, people say exactly the wrong things. These are a handful.

 1. "I know exactly how you must be feeling. My dog/cat/friend's uncle's cousin had cancer and it was just so hard for me to watch."

No. Don't you dare tell me you know what it's like. Especially not when your only experience is with someone you see once a year. A trusted friend once turned the tables on me while out to lunch after asking me about Dad, and I was left to comfort her while she cried over the loss of her dog.

And I get it, animals are family. But a dog is NOT the same thing as a dad. And don't try to tell me otherwise.

2. "Have you prayed about it?"

This is basically the Christian version of "Have you tried turning it on and off again?" Don't do it. It's insulting and rude, well-meaning as it may be. Of course I've prayed. You don't think I've watched my father suffer for three years and never once tried to petition the Almighty to have it all cease? The fact of the matter is that, yes, I have. For a long time, in fact.

And it's just. not. working.

3. "Just have faith."

This one is also quite rude and insensitive. If all it took was faith to make my dad well again, we'd have seen the end of this a long time ago. Yes, faith is part of the medicine. Faith in the doctors, faith in the drugs, faith in God at the end of it all. But a patient does not heal by faith alone. There's chemotherapy. There's radiation. Diet. Exercise. Surgery. All of which are excruciating to watch someone work through.

Cancer isn't an easy problem to solve. Please, please don't give me an easy solution.

Mammography is a diagnostic technique for examining human breast using low-energy i.e. around 30 kVp X-rays. Mammograms are x-ray images of the breast, which are used to detect breast cancer in women at an early stage. This is usually before the disease could show any symptoms or in case of a lump or any other change in the breast.

Young as well as older women in developed countries have been recommended regular mammograms for years for early detection of the cancerous disease. Since, early detection can help cut the number of deaths that occur due to breast cancer, especially in the age group of 40 to 70 years. Also, a mammogram has been proved promising to show tumor growth as compared to a physical exam.

But, despite a huge research and funding backing the subject matter, varied experts in medical communities have countered mammography introduced by mammography machine manufacturer.
